The main distinction
Payment for a physician’s or other health professional’s work, such as performing a procedure, interpreting a study, or providing anesthesia. More than one professional may be involved.
Payment for the hospital outpatient department, ambulatory surgical center, or other facility resources used for the service. It may cover space, staff, equipment, and facility operations under the applicable billing arrangement.
Whether the components appear separately depends on the service, care setting, payer, and billing arrangement. “No facility fee” also does not mean there are no other components.

Not every procedure uses every component. Unexpected clinical needs can also add services that no pre-service estimate could anticipate.
Site of care
An office, ambulatory surgical center, and hospital outpatient department can have different billing and payment structures even when the procedure name is similar. Medicare’s official Procedure Price Lookup compares national average costs for certain outpatient procedures in ambulatory surgical centers and hospital outpatient departments and shows estimated Original Medicare patient payments.
That tool is useful for a national site-of-care comparison, but it is not a personalized quote. It may not include physician fees or every related service. Ask for the exact facility name and address because departments associated with the same health system may bill differently.
What our data covers
MedCostCompare uses the CMS Medicare Physician & Other Practitioners provider-and-service release published in 2026, covering services delivered in calendar year 2024. It is organized by provider, billing code, place of service, service volume, submitted charges, Medicare-approved amounts, and Medicare payments.
Our local provider comparisons use qualifying clinician provider-service records. When a direct organization record is classified by CMS as an Ambulatory Surgical Center, a page may also show its ASC facility amount in a separate table. Other organization records are not labeled as facility amounts. Neither component is a bundled total for every organization involved in the procedure.
Example: A colonoscopy clinician amount may not include the endoscopy facility, anesthesia professional, pathology work if tissue is removed, or other related services. The same principle applies to imaging, surgery, and many diagnostic procedures.
